This form should be completed by an employee claiming for unfair dismissal and, or severance.

To claim for unfair dismissal, an employee should have completed probation or been employed for at least three consecutive months with the employer prior to being terminated.

An employee claiming for severance should have completed at least twelve consecutive months with the employer prior to being terminated.
